HNE Fitnes Promotion
Having trouble sticking to a fitness routine? Why not join a fitness club. Already a gym member? Here’s some incentive to keep up the good work! When you or a covered member of your family purchase a membership to any qualified health club and remain a member for at least 4 consecutive months during the calendar year, HNE will reimburse you up to $150 per family per calendar year.
To be eligible for reimbursement, the health club that you choose must include both cardiovascular and strength-training exercise equipment.
HNE Weight Watchers® Reimbursement Program
Introducing our HNE Watchers® Reimbursement Program! When you or a covered member of your family registers and participates in weekly Weight Watchers® Traditional Meetings or Weight Watchers At Work® meetings, HNE will reimburse you up to $150 per calendar year.
Note: This program does not include Weight Watchers On-line® or Weight Watchers At Home®. Reimbursement is for the cost of the Weight Watchers® Traditional Meetings or Weight Watchers At Work® meetings only and does not include fees paid for food, books, videos or any other items or services. This program does not include fees paid to any other weight loss program.
The maximum reimbursement is $150 per family per calendar year. The reimbursement amount is a combined maximum of $150 for the Weight Watchers® Reimbursement Program and the Health Fitness Promotion Program. In other words, the amount that HNE will reimburse you for the new Weight Watchers® Reimbursement Program, the HNE Health Fitness Promotion, or any combination of the two will not exceed $150 per family per calendar year.
How Do I Submit a Claim for Reimbursement?
To request reimbursement, please submit all of the following:
For HNE WEIGHT WATCHERS® REIMBURSEMENT PROGRAM:
- A completed, signed copy of the HNE Weight Watchers® and Fitness Reimbursement Form.• A completed, signed copy of the
HNE Weight Watchers® and Fitness Reimbursement Form.
- A copy (front and back) of your stamped Weight Watchers® Membership Book.
(Please feel free to black out your weight.)
- Proof of payment (receipts or copies of bank or credit card statements). Proof of payment must include the member’s name and charges for Weight Watchers® sessions.
For HNE FITNESS PROMOTION:
- A completed, signed copy of the HNE Weight Watchers® and Fitness Reimbursement Form.
- A copy of the health club contract or membership agreement and dated, original receipts from your health club, or copies of bank or credit card statements for electronic fund transfer payments. The receipts must include the member’s name and the individual charges for each health club membership.
- HNE will reimburse only the amount you have spent at the time of your submission. For example, if your fitness club membership is $25.00 per month, at the end of the four months you will have spent $100. To be eligible for up to $150.00 please submit your reimbursement form after you have spent the full amount. In this example that would be after 6 months.
who is eligible for these programs?
For HNE WEIGHT WATCHERS® REIMBURSEMENT PROGRAM and HNE FITNESS PROGRAM:
- You must be an active HNE member when you request reimbursement.
- You can submit only one reimbursement form per family in each calendar year.
- You must file your request by March 31st of the following year.
For HNE FITNESS PROMOTION:
- You must be an active member of HNE and the fitness club for at least 4 consecutive months during the calendar year.
For example:
Your HNE coverage starts on July 1, 2008. HNE will reimburse you only for fitness club membership fees incurred after July 1, 2008. If you stay enrolled in HNE and the fitness club for 4 consecutive months (until November 2008) you will be eligible for reimbursement. Your reimbursement request must be submitted by March 31, 2009. At the time you submit your request, you must be an active member of HNE.
Be sure to keep a copy of your receipts before sending in the originals.
Receipts and/or copies of contracts will not be returned to you.
