Disease Management Programs:  Asthma

Adult and Pediatric Asthma Management Program

At HNE, we understand how difficult it can be to manage your asthma.  We also know that having asthma can interfere with your full enjoyment of life.  It may cause you to miss days from work or school and limit your activity.  That is why HNE has developed the Asthma Management Program. 

This program is designed for adults with asthma or the parents of children with asthma.  Through a series of 4 coaching sessions over a month period, the program staff works with you to determine your education needs and struggles.  You will work with your case manager to develop control over your asthma and a strong relationship with your physicians.  The program is available to you free of charge. 

 As part of the program you will:

 

  • Have a one on one relationship with a care manager.
  • Receive education over the phone with supplemental materials mailed to you.
  • Receive guidance on issues such as smoking, triggers, exercise, working with your health care provider, using your medications, etc. 

 

If you would like to participate in our program, please call 1-800-842-4464 and ask a Member Services Associate to connect you with the Care Manager for you.

 


Regardless of whether you choose to work with your personal Care Manager or not, you can benefit from the great resources provided to you here below!  Please take the time to educate yourself and gain more control over your asthma.

Asthma Control Test:  How bad is your asthma?

HNE encourage all people who have or think they have asthma to print this test (linked here below), complete it and take it with you to your/your childs next doctor appointment.  Review the results of your test with your doctor at that appointment.

 

Asthma Action Plan:  Do you have a plan? 

It's important that you have a plan.  HNE encourages all people with asthma to print this plan (linked here below) and take it with you to your/your childs next doctor appointment (along with the results of your Asthma Control Test) and develop a plan together.
