At this Web site, you’ll find:

• A personalized health survey,
• Helpful forms for your doctor or nurse,
• Useful informational links to other on-line resources
• And much more.

The survey was developed at Dartmouth Medical School and is completely confidential and anonymous.

Once you complete the survey, you’ll get a summary of your responses. You’ll also get links to information that focuses on your health concerns. If you are living with a chronic disease, you’ll get a list of important questions to ask your doctor. A “summary action form” describes your most important concerns - making it easier to talk to your doctor about them.

We encourage you to take the survey and bring the summary action form to your next doctor’s appointment.

Taking the survey.

• Once you get to the Web site, click on “Step One: Begin Your Health Check-up.”
  
  
• Click on “Click here if you do not have a code but would like to do the survey.”
  
  
• You and your family members can complete the survey by selecting the appropriate age and sex. All of your answers are confidential - no name identification is required. Based on your answers, you will be given special information personalized to your needs.
  
  
• When you’re done, you will receive a summary action form and other forms with your results. Please, share this with your doctor or nurse.
