In the past, HNE provided PCPs with DM panel reports that listed their patients identified as having diabetes, asthma, and high cholesterol.  PCPs were asked to confirm the patients’ diagnosis and, because HNE did not have access to electronic lab results, provide A1C and LDL lab values. 

We are happy to announce that, because most of our contracted laboratory facilities are now providing lab values to HNE, we no longer will send DM panel reports to ask for this information.
