Recently HNE designed and introduced a form for use by our members, the HNE Personal Health Record. The intent of the form is to provide members with a convenient way to document their health information, and to maintain that information collectively. We created two forms, one for adults and one for children and adolescents and we encourage members to fill one out for themselves and each family member. We have also suggested that members bring the form with them to every health care visit.
The HNE Personal Health Record includes the following information:
- Personal information;
- Emergency contacts;
- Insurance information;
- Names of the member's health care providers;
- Prescription medications;
- Medical and surgical history overviews;
- Immunization history.
The form also encourages members to ask the following three questions at every healthcare visit.
- What is my main problem?
- What do I need to do?
- Why is it important for me to do this?
